Welcome to Mail and More of Dolan Springs! We are a commercial mail receiving agency (CMRA) located in the beautiful little town of Dolan Springs, Arizona--where cattle and cacti outnumber the people. We offer private mailbox rentals, shipping services, package receiving, stamps and shipping supplies, document services, mail forwarding, and so much more!

In order to receive mail and/or packages at Mail and More of Dolan Springs, all persons aged 18+ years, must provide two forms of identification and complete required paperwork per US Postal Service Regulations. In this section, you will find information on the acceptable forms of identification and find downloadable/printable versions of the required documents. 

Identification Requirements

 

Each adult receiving mail at Mail and More must supply two forms of valid identification. One item must contain a photograph of the addressee. The identification presented must be current. It must contain sufficient information to confirm that the applicant is who s/he claims to be and is traceable to the bearer. The CMRA owner or manager may retain a photocopy of the identification for verification purposes. The CMRA must upload all documents into the USPS Database. Social Security cards, credit cards, utility bills, and birth certificates are unacceptable as identification.

 

The following are the only acceptable forms of photo identification:

 

  1. US State/Territory/Tribal Driver’s License*

  2. US State/Territory/Tribal Nondriver’s ID Card*

  3. Uniformed Services Card

  4. Passport

  5. Certificate of Naturalization

  6. US Access Card

  7. Matricula Consular

  8. US Permanent Resident Card

  9. US University ID Card

  10. Nexus Card

 

The following are the only acceptable forms of address verification documents:

 

  1. US State/Territory/Tribal Driver’s License*

  2. US State/Territory/Tribal Nondriver’s ID Card*

  3. Current Lease

  4. Home or vehicle insurance policy

  5. Mortgage or Deed of Trust

  6. Vehicle Registration Card

  7. Voter Card

 

Utility bills are NOT an acceptable form of address verification.

 

*A driver’s license or nondriver’s ID card can only be used for one form of identification, not both. Two driver’s licenses or nondriver’s ID cards cannot be used.

 

If the applicant is unable to substantiate the address, the CMRA must deny the application. Furnishing false information on the application or refusing to give required information is a reason for withholding the addressee's mail from delivery to the agent and returning it to sender. When any information required on Form 1583 changes, the addressee must file a revised application (write "revised" on the form) with the CMRA.

The forms below are provided for your convenience. Only ONE Mailbox Service Application is needed per box. However, ALL adults that will be receiving mail will need to complete and provide their own Contract Agreement for Private Mailbox Rental and Form 1583.
